Identify those who will represent your organization in WHIE meetings (one representative and alternate if desired).
If your organization will join in more than one member category, please use multiple application forms to identify a different representative/alternate pair for each member category circled above. (Representatives, but not alternates, may run for the Board of Directors. Vacated Director positions are replaced as outlined in the WHIE by-laws.) Indicate on the application whether the representative of your organization is interested in running for a Director position representing his or her category. Representatives of Associations may stand for election representing another category (for example, a hospital association representative may stand for Board election from the Hospital category). Directors are elected to staggered three year terms.
